If you are a current NTU student and wish to request an appointment we require you to fill in this short form.

We ask students to prioritise their appointment with us over other commitments as appointments are in high demand and rearranging will cause delays.

The Mental Health Support Team does not offer a crisis service. If you need to be seen urgently you should arrange to meet your GP, or if you are in immediate danger call an ambulance or go to your nearest Accident and Emergency Department. Data Protection and Confidentiality Statement

Information supplied will be used by the University in accordance with the Data Protection Act 1998. The information that you disclose will be used to process your request for an appointment.

Information supplied by you on this form may be shared with other University staff within Student Support Services to enable the University to provide you with relevant advice and support. The University will not normally share the information you have provided with third parties. However, there may be circumstances where it is necessary to disclose information about you. These circumstances are as follows:

  • When required to do so by law or as a result of a court order
  • If you disclose information which suggests that a child has been or may be harmed
  • If there is reason to believe that you may commit serious harm to yourself or others

Anonymised information about those who use this service is collected and used for service evaluation and statistical purposes.
